Una paciente que recibía corticoides desarrolló una neumonía nosocomial, aislándose únicamente Legionella pneumophila en una muestra obtenida por biopsia aspirativa pulmonar transtorácica. En el día 21 de tratamiento con eritromicina y rifampicina, la radiografía de tórax mostró cavitación pulmonar múltiple y el cultivo de esputo fue positivo para legionella. El cultivo de esputo fue negativo para L. pneumophila después de 31 días de tratamiento antibiótico, apreciándose fibrosis de las lesiones cavitarias en la radiografía de tórax dos meses más tarde. La persistencia de L. pneumophila en las muestras microbiológicas durante el tratamiento es de esperar en la enfermedad cavitaria, por lo que el tratamiento antibiótico debe ser prolongado.

A patient receiving corticoid treatment developed a hospital acquired pneumonia; transthoracic pulmonary aspiration biopsy only cultured Legionella pneumophila. On day 21 of erythromycin and rifampicin treatment, chest films disclosed multiple pulmonary cavities and sputum culture was positive for legionellae. On day 31 of the antibiotic therapy, sputum culture was negative for L. pneumophila and a chest film two months later showed fibrosis of the cavities. The persistence of L. pneumophila during the treatment period may be attributed to the cavities, and antibiotic therapy therefore should be prolonged.
