Hemotórax masivo en el curso del tratamiento anticoagulante en un paciente con tromboembolismo pulmonar

Se describe el caso de un paciente de 55 años afecto de un tromboembolismo pulmonar que desarrolló un hemotórax masivo mientras estaba sometido a tratamiento anticoagulante con heparina y posteriormente con cumarínicos. La TC torácica evidenció la coexistencia de un infarto pulmonar homolateral. No existía sobredosificación de los fármacos anticoagulantes ni ninguna otra enfermedad que justificara el hemotórax. El cuadro clínico mejoró con la supresión del tratamiento anticoagulante y vaciado pleural.

Se discute la patogenia del hemotórax asociado a tromboembolismo pulmonar, los posibles factores condicionantes de su aparición en este paciente y la actitud terapéutica en presencia de un infarto pulmonar con hemotórax masivo.

We describe a 55 years old male patient with pulmonary thromboembolism who developed massive hemothorax during anticoagulant therapy with heparin and later on with dicumarinic drugs. Computerized axial tomography of the thorax evidenced a homolateral pulmonary infarction. There was no overdosification of the anticoagulant drugs and we failed to found other diseases that could explain the mecanism of the hemothorax. The clinical picture improved after withdrawal of the anticoagulants and after pleural drainage.

We discuss the pathogenesis of hemothorax associated with pulmonary thromboembolism, the possible factors favouring the occurrence of this association in our patient, and the therapeutic procedure in cases of pulmonary infarction and massive hemothorax.
