Se comunica un caso de Fístula Arterviosa Pulmonar (FAVP) en un paciente con Telangiectasia Hemorrágica Hereditaria (THH) cuya primera manifestación fue un hemotórax. Se sospechó el diagnóstico clínicamente por la auscultación y registro de un soplo sistólico-diastólico en plano posterior de hemitórax que se hizo perceptible después de la evacuación por toracocentesis. Se confirmó mediante arteriografía. La evolución fue favorable después de una resección en cuña de FAVP.
Se destaca que es una causa infrecuente de hemotórax. Se revisan 14 casos que habían sido comunicados previamente en la literatura.
The authors communicate one case of pulmonary arteriovenous fístula (PAVF) in a patient with hereditary hemorrhagic telangiectasia (HHT). The fírst manifestation was hemothorax. This diagnosis was suspected clinically by auscultation and register of a systolic-diastolic souffle in posterior plañe of hemithorax which became perceptible after evacuation by thoracentesis. The diagnosis was confirmed by arteriography. Evolution was favorable after a wedge resection of the PAVF.
The authors emphasize the fact that this is an infrequent cause of hemothorax. They review the 14 case that had been communicated previously in medical bibliography.
