We report three cases of alveolar microlithiasis (AM) in whom the diagnosis was done on the grounds of transbronchial pulmonary biopsy (TPB). In all the three cases, the chest roentgenograms had revealed a bilateral reticulonodular infiltrate, but the punctiform images of bone density, typical of this disease, could not been observed. The TPB disclosed intraalveolar calcareous concretions which are characteristic of alveolar microlithiasis. We emphasize the usefulness of the TPB for diagnosing this entity, particularly in those patients whose chest roentgenograms do not suffice for establishing the diagnosis.
