Resumen

La función pulmonar unilateral (FPU) se estudió en 25 pacientes respiratorios mediante la espirometría en decúbito lateral (EDL) según descripción de Bergan.

La EDL, fundamentada en las variaciones de la capacidad residual funcional (FRC) al pasar del decúbito supino al lateral, refleja la distribución relativa de la función entre los dos pulmones. Los resultados así obtenidos se compararon a los de la cuantificación unilateral de la perfusión pulmonar por gammagrafía con microesferas de 99mmTc. La media del porcentaje del pulmón derecho de los 25 pacientes fue 61,2 ± 21,50 % según la EDL, y de 60,9 ± 18,8 % por gammagrafía. El coeficiente de correlación entre las dos técnicas fue 0,902 y la t de Student de la diferencia entre las medias no fue estadísticamente significativa.

La determinación de la FPU con la EDL es sencilla, requiere equipo simple y ofrece buena correlación con la gammagrafía. A falta de ésta, merece incluirse en el estudio funcional pulmonar de los pacientes con patología de predominio unilateral, particularmente en la valoración previa a la neumonectomía. No obstante, la información respecto a su variabilidad y limitaciones es hasta el momento incompleta.

Summary

Unilateral pulmonary function (UPF) was studied in twenty-five patients with respiratory disease according to Bergané decubitus lateral spirometry technique (DLS).

The relative distribution of function between the two lungs is reflected by DLS, mainly through functional residual capacity (FRC) when the patient changes from supine to lateral decubitus. The obtained results were compared with those from unilateral measurement of pulmonary perfusión by 99m Te microspheres gammagraphy. The mean for the right lung was 61.2-21.50 % with DLS and 60.9-18,8 % with gammagraphy in all patients. Correlation coefficient for the two methods was 0.902, deviation from the mean was not statistically significant when the Students t test was applied.

UPF determination with DLS is a simple procedure, requiring simple equipment and holding a good correlation with gammagraphy. It is a good procedure when gammagraphy is not feasible, and merits inclusión in the pulmonary function studies for patients with predominantly unilateral pathology, especially as a pre-pneumonectomy evalution. However, information on it’s limitations and variability is not yet complete.
