Toracotomía por carcinoma broncogénico en pacientes mayores de 70 años

En este trabajo se analiza, de manera retrospectiva, el resultado del tratamiento quirúrgico de 27 pacientes de 70 o más años, diagnosticados de carcinoma bronquial y estudiados dentro de un protocolo prospectivo. La mortalidad global de la serie es del 11% (tres pacientes) y la probabilidad de supervivencia a los cinco años, por el método actuarial, del 42%. En los pacientes operados en estadios I y II, el beneficio de la cirugía supera al riesgo operatorio. En estos estadios, la mortalidad operatoria es del 0% y la supervivencia real a los tres años, del 40 y 30%, respectivamente. En estadio III-a, el riesgo de muerte operatoria (27%) supera a la supervivencia, por lo que se sugiere que, en estos pacientes, se adopte un tratamiento no quirúrgico de su cáncer de pulmón.

In this work we analized, retrospectively, the result of surgical treatment in 27 patients older than 70 years diagnosticated by bronchogenic carcinoma and studied in a prospective protocol.

The overall mortality is 11% (3 patients) and 42% probability of survival in 5 years by the actuarial method.

The profit of surgery is higher that surgical risk in I and II stages patients. In this stages, the surgical mortality is 0% and, 40% and 30% the real survival in 3 years respectively. In III-a stage the risk of surgical death (27%) surpass the survival, from what it is recommended a non-chirurgical carcinoma of the lung treatment in this patients.
