Hemos estudiado de forma prospectiva 45 pacientes consecutivos con sospecha de tromboembolismo pulmonar (TEP), a los que se realizó una flebocavografía de contraste en las primeras 72 horas. De los 45 pacientes, 24 tenían TEP (grupo I) y en 21 se descarté esta posibilidad (grupo II). En el grupo I un 75% mostró trombosis venosa profunda (TVP) frente a un 19% en el grupo II (p < 0,001). El límite superior de la TVP afectó al sector ilíaco en cuatro pacientes y a la vena cava en dos. Las cuatro trombosis de localización distal (por debajo de poplítea), tuvieron en nuestro estudio una significación patológica, pues todas pertenecían a pacientes del grupo I. La presencia de TVP supuso una sensibilidad del 75% y una especificidad del 81% para el diagnóstico del TEP. En el 16,6% de los pacientes con TEP, la flebocavografía indicó la colocación de un filtro de cava. A nuestro juicio, en los pacientes con sospecha de TEP, la valoración del sistema venoso profundo mediante flebocavografía, tiene importantes implicaciones diagnósticas, pronosticas y terapéuticas.

Forty-five consecutive patients with suspicion of pulmonary thromboembolism (PTE) and in whom a contrast phlebocavography was carried out in the first 72 hours were prospectively studied. Out of the 45 patients, 24 had PTE (Group I) and in 21 patients this possibility was discarded (Group II). In Group I, deep vein thrombosis (DVT) was observed in 75% of the patients versus 19% in Group II (p < 0.001). The upper limit of the DVT involved the iliac/area in four patients and the vena cava in two. The four thrombosis of distal location (below the popliteum) showed pathologic significance in this study given that all pertained to group I patients. Presence of DVT gave sensivity of 75% and specificity of 81% for the diagnosis of PTE. In 16.6% of the patients with PTE the phlebocavography indicated the placement of a cava filter. The authors consider that in patients suspected of having PTE the evaluation of the deep venous system by phlebocavography has important diagnostic, prognostic and therapeutic implications.
