Infección respiratoria por Pasteurella multocida en un paciente con síndrome de lóbulo medio asociado

Se presenta el caso de una paciente de 68 años que vive en medio rural y tiene criterios de EPOC. Al mes de ser diagnosticada de neumonía de lóbulo inferior izquierdo en otro centro hospitalario, se apreció en un control radiológico una imagen compatible con síndrome de lóbulo medio, coincidiendo con una clínica anodina de astenia, anorexia, febrícula vepertina y expectoración verdosa. En la fibrobroncoscopia se le aspiraron abundantes secreciones mucopurulentas en las que se aisló en cultivo puro Pasteurella multocida. Antes de conocer el resultado del cultivo se inició un tratamiento con cefadroxi-lo matenido 20 días hasta su revisión. Ante la persistencia de síntomas y aislamiento del germen en el esputo se cambió el tratamiento a penicilina durante 30 días, consiguiéndose la mejoría clínica y erradicación de P. multocida. Se insiste en la posibilidad de diagnosticar infección pulmonar por dicha bacteria en pacientes que presentan EPOC y conviven con animales.

We report the case of a 68 years old female patient who is living in a rural area and has clinical criteria of chronic obstructive pulmonary disease. One month after being diagno-sed of pneumonia of the left inferior lobe in another hospital, we observed a radiologic image suggestive of middle lobe syndrome during a radiologic examination performed because she presented nonspecific clinical picture of asthenia, anorexia, vespertine febricula, and greenish expectoration. During fibrobronchoscopy abundant mucopurulent secretions were as-pirated leading to isolation of a pure culture of Pasteurella multocida. Treatment with cefradoxil was initiated before knowing the results of the culture and it was maintained during a period of 20 days. In view of the persistence of the symptoms and of the isolation of the microorganism in the sputum we changed the treatment and penicillin was adminis-tered during a period of 30 days. The latter resulted into a clinical improvement and erradication of Pasteurella multocida. We insist on the possibility of infection by Pasteurella multocida in patients with chronic obstructive pulmonary disease who live together with animals and present pulmonary infection.
