Con la finalidad de determinar posibles variaciones en la gasometría arterial (GA) dependiendo del método de conservación y del material de jeringa utilizado, se analizaron las muestras procedentes de 23 pacientes respirando aire ambiente y de 20 enfermos con insuficiencia respiratoria crónica (IRC) recibiendo oxígeno suplementario al 24-28%. De cada enfermo se obtuvieron 6 muestras que se mantuvieron en jeringas de cristal y plástico, tanto a temperatura ambiente como en hielo. La duración de cada estudio fue de 2 horas, realizándose una lectura basal y posteriormente a los 15, 30, 60 y 120 minutos.

Sólo cuando la muestra se mantuvo en jeringa de cristal y hielo, los resultados fueron adecuados a las dos horas postextracción. En el resto, una demora en la lectura superior a 15 minutos produjo variaciones en la PaO2, PaCO2 y pH, que fueron incrementándose de forma progresiva en función del tiempo transcurrido.

Es de resaltar la magnitud de los cambios observados, con diferencias al final del estudio de 9,4 y 10 mmHg en la PaO2 de los dos grupos, diferencias que por sí mismas pueden condicionar importantes cambios en el diagnóstico y manejo clínico, especialmente del paciente con IRC.

To determine possible variations in the values of arterial blood gases depending on the preservation procedure and on the characteristics of the syringe used, the samples obtained in 23 patients breathing ambiental air and in 20 patients with chronic respiratory failure receiving oxygen at 24 to 28%, were analyzed. In each patient 6 samples were obtained and were collected with crystal or plastic syringes at room temperature and under ice. The duration of the study was about 2 hours. Samples were analyzed at baseline and after 15,30,60, and 120 min. Samples collected with crystal syringes and those kept under ice gave the more concordant values during the two hours that followed the extraction. In the remaining situations a delay of 15 min after extraction produced variations in the values of PaO2, PaCO2, and pH. These variations increased with the time. At the late stages of the study, differences of 9.4 to 10 mmHg in PaO2 could be seen. This variability may be of clinical relevance in the diagnosis and treatment specially in patients with chronic respiratory failure.
