A pesar de la publicación reciente de diversas normativas sobre el tratamiento de la bronquitis crónica y la enfermedad pulmonar obstructiva crónica, no existen trabajos dedicados a estudiar su implantación en diversos ámbitos asistenciales. El presente estudio intenta conocer las pautas de tratamiento administradas más frecuentemente a pacientes con bronquitis crónica y enfermedad pulmonar obstructiva crónica en atención primaria en España e identificar factores asociados con la prescripción de diversos fármacos.

Se ha obtenido información mediante un cuestionario estandarizado de 268 médicos de atención primaria distribuidos en las 17 comunidades autónomas elegidos mediante un muestreo razonado estratificado por comunidades. Han aportado información sobre 2.414 pacientes válidos: varones (74%); edad media de 67 años (DE=10); 1.130 (47%) disponían de pruebas funcionales respiratorias, la media del FEV1 fue de 1.523ml (el 56% del teórico). Los enfermos sin pruebas funcionales respiratorias a pesar de tener unos marcadores de gravedad clínica similares sufrieron más agudizaciones y generaron más visitas a urgencias y a su médico habitual el año previo que los pacientes con pruebas funcionales respiratorias (p<0,03; p<0,001; p<0,003). Los tratamientos administrados con mayor frecuencia fueron: beta 2 inhalados de corta duración (56%), corticoides inhalados (47%), teofilinas (43%) y beta 2 de larga duración (41%). Los pacientes sin pruebas funcionales respiratorias recibían más mucolíticos (el 47 frente al 27%; p<0,001) y menos anticolinérgicos (el 20 frente al 35%; p<0,001). El factor más intensamente asociado a la prescripción de todos los fármacos fue la gravedad de la enfermedad medida por el grado de disnea.

Las pautas de tratamiento de la bronquitis crónica y la enfermedad pulmonar obstructiva crónica en atención primaria ofrecen diferencias en muchos casos con las normativas actuales. Destaca una extensa utilización de corticoides inhalados y beta 2 adrenérgicos de larga duración frente a una escasa prescripción de bromuro de ipratropio. La falta de datos de pruebas funcionales respiratorias se asocia con un peor manejo farmacológico y mayores marcadores de morbilidad.

In spite of the recent publication of various guidelines for the treatment of chronic bronchitis and chronic obstructive pulmonary disease (COPD), no studies have investigated whether or not they are being implemented by caregivers. Our aim was to determine what treatment protocols for patients with chronic bronchitis and COPD are most commonly applied by general practitioners in Spain and to identify factors associated with the prescription of certain drugs.

A standardized questionnaire was administered to 268 general practitioners working in the 17 autonomous communities of Spain. The stratified sample of respondents was selected to reflect the population of each community. Valid information on 2,414 patients was collected. Men accounted for 74% of the patients. Mean age was 67 years (SD=10). Lung function data were available for 1,130 (47%) and mean FEV1 was 1,523ml (56% of the theoretical reference value). Patients without lung function data in spite of having signs of severe symptoms had suffered more acute exacerbations and generated more visits to the emergency room and to their primary care doctors in the preceding year than had patients who had undergone lung function testing (p<0.03; p<0.001; p<0.003, respectively). The treatments most often prescribed were short-acting inhaled ¡3-2 agonists (56%), inhaled corticoids (47%), theophylline-containing drugs (43%), and long- lasting P-2 agonists (41%). Patients who had not undergone function tests received more mucolytics (47% versus 27%, p<0.001) and fewer anticholinergics (20% versus 35%, p<0.001). The factor most highly associated with prescription of all drugs was severity of disease measured by dyspnea.

Treatment protocols for chronic bronchitis and COPD in general practice in many aspects show differences from current guidelines. Noteworthy is the extensive use of inhaled corticoid therapy and long-lasting β-2 adrenergic agonists and the infrequent prescription of anticholinergics. The lack of lung function data is associated with poor pharmacologic management of disease and higher rates of morbidity.
