Objetivos

Determinar la prevalencia de bronquitis crónica, asma y obstrucción al flujo aéreo (OFA) en la población general de una zona urbano-industrial de Cataluña (Sabadell y alrededores) y su grado de asociación con el tabaquismo activo y pasivo.

Métodos

Estudio transversal sobre una muestra de 642 individuos entre 20-70 años de edad de ambos sexos obtenida a partir del censo poblacional. Se entrevistaron a 576 individuos (participación del 90%) mediante el cuestionario estandarizado de la American Thoracic Society, y a 497 individuos (participación del 77%) se les realizó una espirometría forzada con prueba broncodilatadora.

Resultados

La prevalencia de bronquitis crónica fue de 11,6% (el 21% en varones; el 2,7% en mujeres) y la de asma previamente diagnosticada por su médico de 3,3% (el 2,1% en varones; el 4,4% en mujeres). La prevalencia de diversos síntomas relacionados con el asma fue más alta. Así, el 10,6% de la población (el 11,4% en varones; el 9,8% en mujeres) referían haber presentado crisis de disnea con sibilancias y el 38,2% (el 45,4% en varones; el 31,4% en mujeres) refería sibilancias en el último año. La prevalencia de OFA (definida como un FEV1 < 80% con un índice FEV1/FVC < 70%) en el conjunto de la población fue 7,2% (el 10,4% en varones; el 4,1% en mujeres), pero en los varones de 60-70 años de prevalencia fue de 30,8%. La exposición pasiva al tabaco se asoció con la presencia de tos crónica (Or = 4,6) y sibilancias (OR = 1,8), pero no con alteraciones espirométricas significativas.

Conclusiones

La prevalencia de síntomas respiratorios y obstrucción al flujo aéreo es alta en nuestro medio, sobre todo en varones y en el grupo de mayor edad (60-70 años). Además de los fumadores activos y ex fumadores, también los fumadores pasivos tienen mayor riesgo de presentar síntomas respiratorios.

Objectives

To determine the prevalence of chronic bronchitis, asthma and airflow limitation in the general population of the urban-industrial área in and around the town of Sabadell (Catalonia, Spain), and to assess its degree of association with smoking and exposure to secondhand smoke

Methods

Transversal study of 642 men and women between 20 and 70 years of age listed on census rolls. The standardized questionnaire of the American Thoracic Society was used to interview 576 subjects (90%). Tests of torced spirometry with bronchodilation were administered to 497 individuáis (77%).

Results

The prevalence of chronic bronchitis was 11.6% (21% of men and 2.7% of women). Asthma had been diagnosed by a subjecfs doctor in 3.3% of the sample (2.1% of men and 4.4% of women). The prevalence of several asthma-related symptoms was higher. Dyspnea with wheezing was reported by 10.6% ofthe population (11.4% ofmen and 9.8% of women); wheezing within the past year was reported by 38.2% (45.4% of men and 31.4% of women). The prevalence of airflow limitation (defined as FEV1 < 80% with an FEV1/FVC Índex < 70%) in the population sample was 7.2% (10.4% of men and 4.1% of women); in men aged 60 to 70 years, however, the prevalence was 30.8%. Exposure to secondhand smoke was related to the presence of chronic coughing (OR = 4.6) and wheezing (OR = 1.8), but not to significant spirometric changes.

Conclusions

The prevalence of respiratory symptoms and airflow limitation is high in our área, particularly among men and oider patients (60 to 70 years oíd). Like active smokers and ex-smokers, subjects who have been exposed to secondhand smoke are at greater risk of developing respiratory symptoms.
