The results for 207 pleural biopsies (PB) with the Abrams needle in 183 patients are evaluated. The pleura was not encountered in 4.3% of the biopsies. Noter-worthy complications included pneumothorax in a ure-mic patient and pneumothorax requiring drainage in another. A diagnosis of tuberculosis was established in 76% of the PB (24% Ziehl positive). Löwenstein culture of the PB did not add to the positive results and the ra-tio for PB culture or positive fluid or both whether with or without granulomas, suggests negative results in the presence of non-specific PB (X2, p < 0.02). Epithelial neoplasms were diagnosed in 56% of the PB. In 3 cases immunohistochemistry was useful for the diagnosis of mesothelioma. Repeated PB significantly increased positive results.
