Resumen

Se presentan 20 pacientes operados de bullas de enfisema clasificados en dos grupos según su extensión. Tras señalar la disnea como síntoma predominante y responsable de la indicación quirúrgica en la mayoría de nuestros pacientes, se expone la sistemática de estudio y tratamiento. En el capítulo de resultados hay que destacar la ausencia de mortalidad operatoria en nuestra serie. Si bien la evolución clínica postoperatoria de los pacientes encuadrados en el Grupo I —enfisema localizado— es muy buena, en el Grupo II —enfisema difuso— los resultados a corto plazo son buenos para ir deteriorándose paulatinamente debido al carácter evolutivo de la enfermedad de base. En la discusión hemos pretendido plantear la difícil problemática de la indicación quirúrgica y aportar una serie de criterios que creemos importantes a la hora de valorar un presunto candidato al tratamiento quirúrgico.

Summary

The authors present the cases of 20 patients operated on for bullae of emphysema and who were classified in two groups according to the extension. After indicating dyspnea as the predominating symptom and responsible for the surgical indication in the majority of their patients, the authors describe their system of study and treatment. In the chapter on results, the absence of operative mortality in this series is worthy of note. Although the clinical postoperative evolution of the patients included in Group I - localized emphysema - is very good, the short term results in Group II - diffuse emphysema - are good, but deteriorate slowly due to the evolutive character of the basic disease. In the discussion the authors have tried to show some light on the difficult problem of surgical indication and contribute a series of criteria that they feel are important when evaluating a possible candidate for surgical treatment.
