La toracotomía bilateral transesternal (incisión clamshell) es una vía de abordaje que permite una amplia exposición de ambos hemitórax.

Su utilización data de los tiempos iniciales de la cirugía cardíaca, si bien terminó cayendo en desuso ante las ventajas de la esternotomía media.

Con motivo de la cirugía de trasplante pulmonar se ha producido una revitalización en su empleo. Aportamos a continuación 2 casos de neumotorax bilateral abordados quirúrgicamente por esta vía.

Transsternal bilateral thoracotomy (“clam shell” incision) is an approach that allows wide access to both sides of the thorax. Its use dates from the earliest period of cardiac surgery, and it fell into disuse in the face of the advantages of a mid-level sternal approach. The “clam shell” incision was revived, however, with the introduction of lung transplantation. We describe 2 cases of bilateral pneumothorax treated by this surgical route.
