The incidence of pneumothorax in patients with acquired immunodeficincy syndrome (AIDS) and pneumonia by Pneumocystis carinii has experienced an increment which has been favoured by prophylactic treatment with inhaled pentamidine. We describe the case of a patient who developed bilateral pneumothorax and required surgical treatment. We detail the anatomopathologic findings and discuss the therapeutic alternatives.
