Resumen

Se presenta un estudio retrospectivo de 17 casos de infecciones pulmonares y pleurales de etiología anaerobia que incluyen neumonías, neumonías necrotizantes, abscesos pulmonares y empiemas pleurales. Sólo hay un caso de neumonía intrahospitalaria. Las muestras para el estudio bacteriológico se obtuvieron por punción transtraqueal, punción pleural, punción transtorácica y punción de un absceso cerebral metastásico. En tres casos existía asociación con gérmenes aerobios. Se hace un estudio detallado de las características clínicas y radiológicas, así como de los factores predisponentes en este tipo de infecciones. Dos pacientes presentaron un exantema alérgico frente a la clindamicina. Se valoran los resultados del tratamiento antibiótico y la necesidad de recurrir al drenaje cerrado continuo e incluso a la resección costal con ventana torácica en los casos de empiemas pleurales.

Summary

The authors present a retrospective study of 17 cases of pulmonary and pleural infections with anaerboic etiology that include pneumonias, necrotizing pneumonias, pulmonary abscesses and pleural empyemas. There is only one case of intrahospitalary pneumonia. Samples for bacteriologic study were obtained by transtracheal puncture, pleural puncture, transthoracic punture and puncture of a metastasic cerebral abscess. There was association with aerobio germs in three cases. A detailed study is made of clinical and radiologic characteristics as well as of predisposing factors in this type of infections. Two patients presented allergic exanthema to clindamycin. Results of antibiotic treatment are evaluated as well as the necessity of recurring to continuous closed drainage and even costal resection with thoracic window in the cases of pleural empyemas.
