En base a una serie de 22 pacientes afectos de hemoptisis masivas, los autores correlacionan la naturaleza de las lesiones anatomopatológicas con la conducta diagnóstica, tratamiento efectuado y resultados obtenidos.

Las secuelas tuberculosas y las malformaciones vasculares broncopulmonares ocupan los primeros lugares en la génesis de la hemoptisis. Su topografía y las modificaciones parenquimatosas, vasculares, bronquiales y pleurales originadas por las mismas, permiten establecer, tanto su relación con distintos patrones radiológicos, como el rendimiento de los métodos de diagnóstico por imagen e instrumentales.

La gravedad y persistencia del sangrado justifican el tratamiento quirúrgico urgente. Obtener la hemostasia temporal mediante maniobras endoscópicas o embolización arterial bronquial, permite una terapéutica precisa, basada tanto en las características de la lesión responsable como en la repercusión funcional que ésta o su tratamiento puedan conllevar.

Based on a series of 22 patients with massive hemoptysis the authors attempt to correlate the nature of the anatomopatho-logical lesions with the diagnostic procedure, appropiate treatment, and with results of the treatment. Tuberculous sequelae and bronchopulmonary vascular malformations are the two major causes of hemoptysis. The localization and the parenchymal, vascular, bronchial and pleural changes originated by the hemoptysis, allow to establish not only its relationship with different radiologic patterns but also the yield of instrumental and imaging diagnostic procedures. The severity and persistence of the bleeding justify urgent surgical treatment. Temporal hemostasia obtained by means of bronchoscopic manoeuvres or by bronchial arterial embolization allowed to achieve a precise therapy based on the characteristics of the responsible lesion and on functional consequences that hemoptysis and its treatment may carry out.
