Las roturas traqueobronquiales representan una patología muy grave, difícil de diagnosticar en un primer reconocimiento. Se revisan 6 casos: cuatro como formas agudas con neumomediastino, neumotorax y enfisema subcutáneo -con un retraso en el diagnóstico de 3,25 días-, mientras que dos corresponden a formas crónicas -con un retraso medio en el diagnóstico de 124,5 días-.

El diagnóstico se basa en la presencia de neumotorax uni o bilateral no resuelto asociado a neumomediastino, confirmándose la sospecha tras la práctica de la fibrobroncoscopia. El tratamiento habitual consiste en el desbridamiento de la zona lesionada seguido de broncopiastia término-terminal con suturas reabsorbibles. En aquellos casos en que no sea posible se deberá realizar resección pulmonar.

Tracheobronchial ruptures represent a serious pathology difficult to diagnose at the first examination. The authors review 6 cases: four as acute types with pneumomediastinum, pneumothorax and subcutaneous emphysema with a delay in diagnosis of 3.25 days, meanwhile 2 cases were chronic forms with a delay in diagnosis of 124.5 days.

Diagnosis shouid be performed as soon as possible based in the presence of uni or bilateral pneumothorax with pneumomediastinum being confirmed by fiberbronchoscopy. The treatment is based in the resection of the fractured fragments, followed by bronchoplasty always with reabsorbable sutures the most frequent surgical technique, meanwhile in the atelectasic forms it is not possible to perform sometimes and we must practise lung resections.
