Introducción
La enfermedad pulmonar obstructiva crónica (EPOC) comporta una elevada morbilidad y mortalidad en los países industrializados, que se espera aumente en los próximos años1., 2., 3.. Pacientes y métodos
Se estudiaron prospectivamente todos los pacientes que ingresaron en los Servicios de Neumología y Medicina Interna del hospital Mutua de Terrassa por exacerbación de EPOC entre junio de 1998 y junio de 1999.
Los criterios de inclusión eran: a) diagnóstico clínico de EPOC; b) FEV1 post-broncodilatador al alta inferior al 70% de su valor de referencia y FEV1/FVC inferior a 0,7 y c) haber precisado de, al menos, un ingreso por EPOC en el año previo. Se definió exacerbación como un incremento de la
Variables
En el momento del ingreso se realizó una gasometría respirando aire ambiente. Durante la hospitalización se evaluaron las siguientes variables: IMC, historia de tabaquismo, presencia de cor pulmonale, oxigenoterapia domiciliaria, número de ingresos en el año previo por exacerbación de la EPOC y número de fármacos que tomaba antes del ingreso. La espirometría con prueba broncodilatadora, el test de marcha de 6 minutos y una nueva gasometría basal se realizaron el día del alta hospitalaria.
Análisis estadístico
Las variables cualitativas se expresan en porcentajes y las cuantitativas en medias y desviación estándar (± DE) en caso de distribución normal, mientras que para las variables temporales se utilizaron medianas y rangos intercuartiles (RIQ; percentiles 25 a 75%). En el análisis bivariante la relación entre el tiempo de supervivencia y las distintas variables continuas se evaluó mediante el modelo de regresión logística de Cox, mientras que para las variables categóricas se utilizaron las curvas
Resultados
Se incluyeron un total de 66 pacientes (65 hombres y una mujer) con una edad media de 71,6 (± 8,9 años). Ocho de ellos no pudieron realizar el test de marcha de 6 minutos,por lo que inicialmente fueron excluidos del análisis del índice BODE. Las características generales de la población estudiada se detallan en la tabla 1.
Para facilitar la comparación con el estudio original de Celli et al se analizó la supervivencia en dos períodos distintos. En el primero de ellos se escogió el 31 de marzo
Discusión
La principal aportación de nuestro estudio es corroborar que el índice BODE es un predictor de mortalidad útil en pacientes reingresadores por EPOC, y que se relaciona mejor que el FEV1 con la supervivencia. Además, la hipercapnia al alta se confirma también como un factor predictor independiente de mortalidad.
La EPOC es una enfermedad compleja que se caracteriza por una amplia heterogeneidad en su presentación clínica y evolución. La gravedad de la EPOC está asociada a la disminución del FEV1
