La pleurolisis consiste en la administración periódica de uroquinasa, previa extracción del líquido de derrame y a través del mismo catéter de drenaje, en pacientes con derrame pleural encapsulado hemorrágico y/o empiematoso.

Los resultados obtenidos en 14 pacientes con derrame pleural loculado de distinta naturaleza, en los que había fracasado la toracostomía, tras la administración de 240.000 UI de uroquinasa en nueve pacientes y de 400.000 UI en los otros cinco restantes, nos han permitido observar la resolución total del derrame en 11 pacientes (78,5%), con persistencia de líquido residual en el resto (21,5%). No se observaron complicaciones importantes, salvo un hidroneumotórax que se resolvió con el tratamiento habitual. El tiempo medio de permanencia del catéter fue de 3,4 días.

La pleurolisis se perfila como un excelente procedimiento coadyuvante en el tratamiento de los derrames pleurales encapsulados, si bien es necesario completar su estudio.

Pleurolysis consists in the periodic administration of urokinase after the extraction of the effusion fluid through the same drainage catheter in patients with encapsulated hemorrhagic and/or empyematous pleural effusion.

The results obtained in 14 patients with pleural effusion of different natures in whom thoracostomy had failed in nine after the admiistration of 240,000 IU of urokinase, and in the remaining five after being given 400,00 IU have allowd total resolution of the effusion to be observed in 11 patients (78.5%) with persistence of residual fluid in the remaiging patients (21.5%). No important complications were observed except for one hydropneumothorax which was resolved by normal therapy. The mean time for the catheter to remain in place was 3.4 days.

Pleurolysis was found to be an excellent coadjuvant procedure in the treatment of encapsulated pleural effusions although further studies are required.
