Se presentan los resultados del tratamiento quirúrgico en 66 casos de cáncer de pulmón intervenidos entre 1978 y 1986. La supervivencia actuarial a los 4 años es del 30%. Se ha analizado el valor pronóstico de 11 factores encontrándo que la invasión ganglionar, la afectación de márgenes o estructuras vecinas, la presencia de linfangitis neoplásica, el tamaño tumoral, el grado de diferenciación y la edad empeoran significativamente la supervivencia. Cuando se aplica un análisis multivariante, con el modelo del azar proporcional de Cox, sólo alcanzan significación pronostica la invasión ganglionar, el grado de diferenciación y el tamaño tumoral. La recurrencia de la enfermedad ocurre, sobre todo en el primer año después de la cirugia, más frecuentemente por metástasis a distancia y especialmente por metástasis cerebrales u óseas.

We present the results of surgical treatment in 66 cases of lung cancer who were operated between 1978 and 1986. The actuarial survival after four years is of 30%. We analyze the prognostic value of 11 factors, and we found that lymph node spread, involvement of margins or adjacent structures, presence of neoplastic lymphangitis, tumoral size, degree of differentiation and age impaired the survival in a significant way. When multivariant analysis is applied, with the model proportional random of Cox, only lymph node spread, degree of differentiation and tumoral size do achieve prognostic value. Recurrence of the disease occurs mainly within the first year after surgical intervention and it is more often due to distant metastases, and particulary, bone or brain metastases.
