Resumen

Se presenta un caso de neumomediastino idiopático en un joven de 18 años de hábito asténico.

El diagnóstico de la entidad es eminentemente clínico y los datos complementarios para certificarlo son: la radiografía de tórax que permite ver el enfisema mediastínico y subcutáneo y el ECG con FCG que objetiva el signo de Hamman.

Existen multiples etiologías del síndrome clínico siendo la más rara la idiopática y la más frecuente la de hiperpresión endotorácica y la yatrógena (diagnóstica o exploratoria).

El pronóstico es benigno pero depende de las circunstancias de aparición y de la etiología, siendo los de hiperpresión los más graves.

Summary

The authors present a cese of idiopathic pneumomediastinum in an eighteen-year-old young man with an asthenic nature.

Diagnosis of the entity is eminently clinical and the complementary data for verifying it are: the radiography of the thorax which allows one to see the mediastinal and subcutaneous emphysema and the ECG with FCG that Hamman's sign.

There are multiple etiologies of the clinical syndrome. The rarest is idiopathic whereas the most frequent are endothoracic hyperpressure and the iatrogenic (diagnostic or exploratory).

The prognosis is benign, but depends upon the circumstances of appearance and of the etiology. The most grave is that corresponding to hyperpressure.
