Dolor postoracotomia: criocoagulacion o/y neurotripsia versus infiltracion intercostal o/y analgesia general

Presentamos un estudio comparativo entre dos grupos de enfermos intervenidos quirúrgicamente mediante toracotomía posterolateral clásica, para los que se utilizaron técnicas de control del dolor postoperatorio diferentes: infiltración intercostal con analgésicos locales y analgesia general pautada para (40) enfermos que constituyen el grupo I, y criocoagulación (CC) intercostal y neurotripsia (NT) para 37 enfermos, que constituyen el grupo II.

Del conjunto de los resultados, merece destacarse que se obtuvieron diferencias estadísticamente significativas en cuanto a la dosis media analgésica facultativa utilizada entre los enfermos grupo I (20,36 amp.) y grupo II (4,6 amp.); en las citaciones referentes al dolor recogidas en la hoja de enfermería, siendo de 3 veces como citación media en el grupo I frente a 0,4 veces en el grupo II; asimismo, fuera del hospital, a los 40 días, 72% de enfermos del grupo I había precisado algún analgésico esporádicamente frente a un 11% de enfermos del grupo II.

Del análisis de nuestros resultados, los enfermos tratados con CC y NT obtienen mejor y más duradera analgesia por un método menos tóxico y peligroso.

Whe present a comparative study between two groups of patients who underwent classical posterolateral thoracotomy and in whom we employed different procedures in order to ameliorate the postoperative pain. The patients were classified into two groups: group 1, consisted of 40 patients who received intercostal infiltration with local analgesic agents and ruled general analgesia, and group 2, consisting of 37 patients, who received intercostal cryocoagulation and underwent neurotripsy.

With respect to the mean dose of the facultative employed analgesic agents, group 1 patients required 20.36 amp. and group 2 patients, 4.6 amp.; the difference was statistically significant; with respect to the quotations on the nurse's record, group 1 patients had a mean of three quotations and group 2 patients, a mean of 0.4 quotations. Forty days after dismissal, 72% of group 1 patients sporadically required some analgesic agents versus 11% of group 2 patients.

We conclude that patients submitted to intercostal cryoagulation and neurotripsy do achieve a better quality and more durable analgesia by means of a lesser toxic and hazardous procedure.
