Resumen

Las curvas de lavado pulmonar empleando medidores rápidos de N2 y CO2 han sido analizadas desde el punto de vista de su aplicación clínica. Las fuentes de error debidas al paciente, observador o al cálculo de las curvas han sido investigadas. Los resultados obtenidos han sido comparados con las tensiones de O2 en la sangre arterial. El efecto de un broncodilatador (Metil-sulfato de thiazinamium, una droga anticolinérgica y antihistamínica) sobre las curvas de lavado ha sido también investigado. Basados en los datos obtenidos se concluye que el análisis de las curvas de lavado pulmonar tienen poca utilidad en la clínica. Y ello por dos razones: I) Las variaciones debidas al paciente y al investigador son mayores que las habitualmente aceptadas en los laboratorios de patofisiología respiratoria para otras investigaciones; 2) La información que proporcionan no está relacionada con la tensión de O2 en la sangre arterial.

Los cambios experimentados por las curvas de lavado pulmonar tras la administración de un broncodilatador podrían ser útiles para el estudio de la adaptación del cociente ventilación-perfusión, lo cual podría tener un cierto valor pornóstico en los enfermos con una enfermedad pulmonar crónica inespecífica.

Summary

The curves of pulmonary lavage using rapid measurers of N2 and CO2 have been analyzed from the viewpoint of its clinical application. The sources of error due to the patient, the observor or to the calculation of the curves were investigated. The results obtained were compared with the tensions of O2 in arterial blood. The effect of a bronchodilator (methyl sulfate of thiazinamium, an anticholinergic and antihistaminic drug) on the curves of lavage were also investigated. Based on the data they obtained, the authors concluded that the analysis of the curves of pulmonary lavage was of little use in clinical practise. This was due to two reasons: 1) the variations due to the patient and to the investigator are greater than what is normally accepted in laboratories of respiratory pathophsiology for other investigations; and 2) the information obtained is not related to the tensions of O2 in arterial blood.

The changes in the curves of pulmonary lavage after administraron of a bronchodilator could be useful for the study of the adaptation of the ventilation/perfusion quotient, which could have a certain prognostic value for patients with a non-specific chronic pulmonary disease.
