El tratamiento quirúrgico del cáncer bronquial N2 (ganglios mediastínicos ipsilaterales positivos) ha sido muy cuestionado en los últimos años, existiendo posturas contradictorias al respecto. Los autores revisan 27 casos N2 resecados, asociando radioterapia y/o poliquimioterapia adyuvantes, obteniendo una supervivencia actuarial de un 24% a los tres años, frente a un 7% a los tres años para los casos irresecables y no intervenidos.

El factor N2 conlleva un pronóstico postoperatorio más grave con respecto al N0 y N1 (p < 0,05). La quimioterapia neoadyuvante ha cambiado en los últimos años el manejo de estos pacientes, aumentando las tasas de resecabilidad y supervivencia.

The surgical treatment of the N2 bronchogenic carcinoma (positive ipsilateral mediastinal lymph nodes) has been recently questioned and controversial positions have been adopted. The authors review 27 cases of N2 which have been surgically resected and treated with the association of radiotherapy and, or, with polychemotherapy. The actuarial survival after three years was 24% for the resected cases and 7% for the nonoperated patients.

The N2 factor carries a more serious postoperative prognosis than N0 and N1 (p < 0,05). The concurrent chemotherapy has improved in the last years the rates of resectability and survival.
