Efectuamos un estudio comparativo de función pulmonar en individuos afectos de neumotorax espontáneo primario y sometidos a distinta opción terapéutica. El grupo 1 fue tratado con reposo y oxigenoterapia. El grupo 2 fue sometido a drenaje torácico. En el grupo 3 se realizó toracoscopia y pleurodesis con clorhidrato de tetraciclina.

Los grupos 1 y 2 no presentaron diferencias significativas con sus valores teóricos expresados en cifras absolutas en ninguna de las determinaciones. En el grupo 3 no hubieron diferencias significativas en FVC, FEV1, FEV1,/FVC, FEF 25-75, MEF 25, MEF 50, TLC y DCO con respecto a sus valores teóricos, a excepción de RV (p < 0,001) y del cociente RV/TLC (p < 0,001). Aplicando el análisis de la Varianza para comparar los tres grupos entre sí con valores expresados como porcentaje de su teórico no existieron diferencias estadísticas significativas. La pleurodesis en el paciente premedi-cado y anestesiado con lidocaína intrapleural fue tolerada sin producir dolor significativo. No se comprobó intolerancia medicamentosa ni infección pleural. No existió recidiva homola-teral en el grupo tratado con pleurodesis en el año de seguimiento.

Concluimos que la pleurodesis con tetraciclinas puede ser empleada en el tratamiento del neumotorax dada su buena tolerancia, disminución del porcentaje de recidivas y de trastornos funcionales con traducción clínica significativa.

We have undertaken a comparative study of pulmonary function in patients with primary spontaneous pneumothorax subjected to different thrapeutic option. Group 1 was treated with oxygen and rest; group 2 underwent a thoracic drainage; and group 3 was subjected to thoracoscopy and pleurodesis with tetracyclin cholorhydre. Group 1 and 2 had no significant differences in the theoretical values of either of the explora-tions expressed as absolute values. In group 3 there were no significant differences in FVC, FEV1, FEV1/FVC, FEF 25-75, MEF 25, MEF 50, TLC and DCO with respect to their corresponding theoretical values except for RV (p < 0.001) and for RV/TLC quotient (p < 0.001). Applaying the analysis of variance for comparing the three groups with values expressed as percentage of the theoretical values there were no statiscically significant differences. Pleurodesis in a premedi-cated patient under anesthesia with intrapleural lidocaine was well tolerated without significant pain. There was no drug intolerance and there were no pleural infections. During a follow-up period of one year, patients treated with pleurodesis did not experience homolateral relapse of the pneumothorax.

We conclude that pleurodesis with tetracycline may be useful in the treatment of spontaneous pneumothorax in view of its good tolerance, low incidence of relapses, and scanty clinically functional disturbances.
