Se presentan 54 casos de pacientes sospechosos de tuberculosis pulmonar, con la particularidad de ausencia de expectoración o investigación del bacilo tuberculoso en esputo reiteradamente negativa. En ellos se realiza fibrobroncoscopia para obtención de muestras.

De los 54 casos se confirma la enfermedad en 38, obteniéndose el diagnóstico mediante fíbrobroncoscopia en 28 casos, lo que supone una rentabilidad del 74%.

La mayor positividad se obtiene con el cultivo del lavado bronquial. Le siguen en eficacia, el estudio anatomopatológico de la biopsia transbronquial y el cultivo de dicha biopsia. La tinción del lavado es menos rentable y el cepillado bronquial puede omitirse.

Se recomienda la realización de una fíbrobroncoscopia en los casos mencionados, como exploración relativamente bien tolerada y prácticamente sin complicaciones.

The authors describe 54 patients with suspected pulmonary tuberculosis, noteworthy for the absence of expectoration or repeated negative results in sputum cultures for the tuberculous bacilli. Sputum samples were obtained by fiberoptic bronchoscopy. The disease was confirmed in 38 of the 54 cases with the diagnosis obtained by fiberoptic bronchoscopy in 28 cases, representing a 74% success rate. Bronchial lavage obtained the highest number of positive diagnoses, followed by pathologic study of transbronchial biopsies and their culture. Staining of the lavage fluid was less successful and bronchial brushing may be ommited.

Fiberoptic bronchoscopy is recommended as the technique of choice for this type of cases, being relatively well tolerated and practically free of complications.
