Las resecciones de pared torácica se llevan a cabo en multitud de patologías. Las técnicas de reconstrucción han variado notablemente desde la introducción de las prótesis sintéticas, la última de ellas la de politetrafluoroetileno (PTFE) (Gore-Tex®). Presentamos la experiencia de nuestro servicio en la reconstrucción torácica con prótesis de PTFE.

Se utilizó este material en 21 pacientes intervenidos por diversas patologías, de los que 13 son reconstrucciones de la pared torácica, cuatro de diafragma y cuatro de pericardio. En los primeros se asoció en 3 casos una mioplastia bilateral de pectoral mayor, en 2 casos una omentoplastia y en uno un colgajo miocutáneo de dorsal ancho.

Como complicaciones se presentaron neumonía en 2 casos y un seroma con fístula cutánea crónica que motivó la retirada de la prótesis a los 9 meses. Hubo 2 fallecimientos, uno a los 19 días de la intervención y otro a los 9 meses, no relacionados con la técnica de reconstrucción.

El seguimiento oscila entre 3 y 54 meses.

En conclusión, recomendamos la utilización de PTFE como prótesis a utilizar en la reconstrucción torácica, asociada a plastias o colgajos miocutáneos.

Thoracic wall resections are performed to treat a wide variety of conditions. Reconstruction techniques have varied considerably since the introduction of synthetic prosthesis, the most recent of which are made of polytetrafluoroethylene (PTFE, or Gore-Tex®). We describe our department's experience with PTFE prosthesis.

PTFE was used in 21 patients treated for various diseases. Thirteen reconstructions were of the thoracic wall, 4 were of the diaphragm and 4 of the pericardium. Three of the 13 thoracic wall reconstructions involved bilateral myoplasty of the pectoralis major, 2 involved omentoplasty and 1 required use of a wide musculocutaneous flap.

Complications included pneumonia in 2 cases and 1 seroma with chronic cutaneous fistula that required removal of the prosthesis after 9 months. Two patients died, 1 after 19 days and the other after 9 months, both as a result of causes unrelated to reconstruction. Follow-up of these patients ranged from 3 to 54 months.

We recommend the prosthetic use of PTFE for thoracic wall reconstruction, along with plasty or musculocutaneous flaps when necessary.
