Se presenta la experiencia en el diagnóstico y seguimiento de 36 pacientes con carcinoma indiferenciado de células pequeñas de pulmón (CPCP) en el Servicio de Clinica Médica, Sección Hematología y Oncología del Hospital Italiano de Buenos Aires, durante el periodo 1978-1983.

La población eran 31 hombres (86,1%) y 5 mujeres (13,9%), con un promedio de edad de 58,6 años. El 88% eran fumadores. La mayoría se presentaron con síntomas pulmonares como expresión directa del tumor (tos y hemoptisis) y/o extensión a estructuras torácicas (síndrome mediastinal, síndrome de vena cava). El 11,1% tuvieron forma de expresión síndromes paraneoplásicos (2 SSIHAD y 2 polineuropatías sensitivo motoras graves).

En 17 casos la enfermedad fue considerada localizada a un hemitórax y en 19 casos diseminada (enfermedad locorregional avanzada y/o metástasis a distancia). Asimismo el 55% de los pacientes eran ambulatorios.

De los 36 casos, 21 fueron valorables en cuanto a respuesta a tratamiento quimio y radioterapéutico. Se lograron cinco remisiones completas, todas ellas en pacientes localizados tratados con esquemas quimioterapéuticos que incluían adriamicina. Hubieron seis remisiones parciales y 10 progresiones, estas últimas se dieron en pacientes diseminados. La duración de la respuesta asl como la sobrevida tiene diferencias significativas entre los grupos localizados, diseminados y progresiones, con P mayor de 0,025.

La sobrevida global de 10 meses para todo el grupo tratado contrasta con la sobrevida referida en la literatura en la evolución natural de la enfermedad (4 meses). Esto hace importante el correcto diagnóstico, la búsqueda de pacientes en estadios localizados, como la aplicación de esquemas de poliquimioterapia con alto grado de respuesta.

The experience in diagnosis and follow-up of 36 patients with undifferentiated small cell carcinoma of the lung at the Hematology Oncology Section of the Hospital de Buenos Aires during 1978-1983 is presented.

They were 31 men (86%) and 5 women (14%). Mean age was 58.6 years which of them to the tumor (cough, hemoptisis) and/or extension of the tumor to thoracic structures (mediastinal or vein cava syndromes).

Four patients presented with paraneoplastic syndrome (2 cases of inadecuate secretion of antidiuretic hormone and 2 with severe sensory-motor polyneuropathy). In 17 cases, disease was localised to one hemithorax and in the remaining 19 it was disseminated. 55% of the patients were ambulatory. Of the 36 patients 21 could be evaluated with radiochemotherapy. In all cases, tumor was localized and treatment included adriamycin. Six patients had partial remissions. Ten progressed and these were cases with disseminated disease. There were significant differences in response to treatment and survival between the two groups (disseminated vs. localized).

An overall survival of 10 months in the group contrasts with shorter survival in other studies and emphasizes the need for a correct, early diagnosis and the use of combined chemotherapy with chances of high response.
