Se presentan cinco casos de histiocitosis X, entidad de causa desconocida y caracterizada por la presencia de una infiltración granulomatosa pulmonar. La clínica es inespecífíca, pero junto con la radiología nos pueden hacer sospechar el diagnóstico. Las alteraciones en la exploración funcional respiratoria son similares a las obtenidas en el resto de las enfermedades intersticiales pulmonares. La fibrobroncoscopia con biopsia transbronquial y más recientemente el lavado broncoalveolar pueden proporcionar el diagnóstico, por lo que en estos casos no será preciso practicar la biopsia pulmonar a cielo abierto. El tratamiento consiste en la administraciónn de esteroides, si bien no ha podido ser demostrada su eficacia. La evolución suele ser favorable.

Five cases are presented of histiocytosis X, an entity of unknown cause and characterized by the presence of granulomatous infiltration of the lung. The clinical features are not specific although together with the radiologic findings may suggest the diagnosis. The alterations in respiratory function tests are similar to those for other interstitial diseases of the lung. Fiberoptic bronchoscopy with transbronchial biopsy and the more recent technique of bronchoalveolar lavage may aid in establishing and avoid unnecessary open lavage biopsy. Treatment consists in steroid therapy although its efficacy has not been demonstrated. The course is usually favourable.
