We comparatively studied a series of 79 patients of spontaneuos idiopathic pneumothorax observed between February 1987 and December 1989 who were treated with a 12 F drainage tube in 40 cases and with a 10 F tube in the remaining patients. Drainage with small diameter tubes was effective and was used during a mean period of 5,2±3,9 days. Only one case required surgical treatment due to persistent air leakage. There were no significant differences among these two groups with respect to the duration of drainage, complications, and relapses during a minimal follow-up of 12 months. Therefore, we recommend the use small-diameter tubes for the treatment of spontaneous idiopathic pneumothorax, specially those of 10 F size since, as compared with 12 F are less traumatic and have a similar effectiveness.
