Aportamos 22 casos de carcinoma bronquioloalveolar diagnosticados en un plazo de 21 años y revisamos el concepto y su relación con otras formas de adenocarcinoma pulmonar. En todos los casos se valoró la historia clínica, la forma de presentación radiológica y la citología de esputo. En 17 se realizó broncofibroscopia, en 14 biopsia por toracotomía y cuatro fueron necropsias.

Encontramos un predominio de varones con una edad media de 57 años. El síntoma más frecuente fue la tos (50%). El patrón radiológico más observado fue el retículo-nodular bilateral (32%), seguido del nodulo pulmonar solitario y neumónico (22,7% cada uno). En todos los pacientes, el diagnóstico se hizo en base al hallazgo de un patrón anatomopatológico considerado típico, en el material de la biopsia transbronquial, por toracotomía o necropsia. Valoramos además la rentabilidad de otros métodos diagnósticos.

We report 22 cases of bronchioloalveolar carcinoma which were diagnosed during a period of 21 years. We revise the concept and its relationship with other types of pulmonary carcinoma. In all cases we evaluated the clinical history, the radiologic form of presentation, and the cytologic examination of the sputum. Fiberoptic bronchoscopy was performed in 17 patients, biopsi through a thoracotomy in 14, and necropsic study in 4 patients. We observed a male sex predominance with a mean age of 57 years. The most frequent symptom was cough (50%). Bilateral reticulo-nodular pattern was the most common radiologic finding (32%), followed by solitary pulmonary nodule (22,7%) and pneumonic pattern (22,7%). In all patients the diagnosis was based on the presence of a typical pathologic pattern in specimen obtained from transbronchial biopsy, thoracotomy, or during the necropsic study. We also evaluated the yield of other diagnostic procedures.
